We are a Federally Qualified Health Center (FQHC), which means we can offer a wide variety of services through a sliding fee discount. The sliding fee discount is available to all income-eligible uninsured or underinsured patients and is applicable to most services. The discount is based on annual household income & family size. To qualify for the sliding fee discount, you must fill out an application and show proof of annual income for all immediate family members living in your household.
